POSITION SUMMARY:

• Provides skilled nursing services under the supervision of RNs in accordance with the established Plan of Care
• Provides direct patient care as defined in the State Nurse Practice Act.
• Contributes and participates in care coordination and patient-centered care as a part of an interdisciplinary clinical care team of nurses, therapists, CNA's and MSW's

QUALIFICATIONS:

• Professional Arizona nursing license in good standing.
• Minimum of one year's experience with a preference for prior Home Health Experience.
• CPR certification.
• Fingerprint clearance card.
• TB test result.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements with a good driving record. 
• Demonstrates strong verbal and written communication skills
