Your Role as a Hospice Nurse:
Delivering Exceptional Patient Care
Provide compassionate nursing care, coordinating services for patients and caregivers.
Conduct thorough nursing assessments to support individualized care plans.
Develop, implement, evaluate, and adjust care plans to ensure seamless, goal-oriented care.
Empower patients and caregivers by promoting independence whenever possible.
Adhere to established hospice care protocols and guidelines.
Educating and Supporting Patients and Families
Deliver effective patient and caregiver education to promote understanding and comfort.
Encourage open communication, providing time for questions and discussions.
Act as a knowledgeable resource to patients, families, and interdisciplinary team members.
Collaborating with the Care Team
Participate in interdisciplinary group (IDG) meetings and special care conferences.
Build and maintain strong relationships with team members to ensure coordinated care.
Identify patient and caregiver needs and make appropriate referrals.
Maintaining Accurate Documentation
Ensure timely, complete, and precise clinical documentation.
Follow licensure and certification standards in maintaining patient records.
Demonstrating Professionalism and Accountability
Stay current with best practices and trends in hospice and palliative care.
Maintain active professional licensure, CPR certification, and continuing education requirements.
Exhibit excellent time management skills and the ability to respond to urgent situations.
Foster positive relationships with external agencies and healthcare providers.
Additional Responsibilities (For Inpatient Unit Nurses)
Maintain a clean and well-stocked patient environment.
Follow infection control protocols and appropriate cleaning procedures.
Ensure the comfort of families in shared spaces.
Qualifications:
Diploma or Associate Degree in Nursing required; Bachelor's Degree preferred.
Active, unrestricted Registered Nurse (RN) license in a compact (multistate) or applicable state.
Current CPR certification required; CHPN certification preferred.
Minimum 1 year of experience for inpatient unit (IPU) nurses; 2 years for other nursing roles.
Direct patient care experience with life-limiting illnesses required (hospice, palliative care, geriatrics, oncology, home health, etc.).
Strong computer proficiency.
