Our client is seeking a Master level Licensed Social worker to work with adults in a Long term care facility in the Hagerstown, MD area. This is a full schedule, 8:00 - 5, Monday-Friday assignment. Potential for hybrid schedule - 3 days on site/2 days remote.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Identify individual social and emotional needs
* Develop and maintain individualized care plans
* Participate in the planning of the resident's admissionand discharge to the community
* Coordinate investigations for resident grievances

CLIENT'S REQUIRED SKILLS AND EXPERIENCE
* Master degree in Social Work
* Maryland social worker license, LCSW preferred
* Long-term care experience preferred
