Job Description
Seniority Healthcare is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team. In this role, you will provide compassionate, comprehensive care to patients, primarily in senior living communities. You’ll enjoy the autonomy of managing your patient panel while working in close collaboration with a supervising physician.
Key Responsibilities
- Conduct thorough physical exams and wellness assessments for seniors.
- Develop personalized care plans and coordinate all aspects of care, including labs, follow-ups, and care team communication.
- Act as the primary liaison between ancillary providers, facility staff, and Seniority Healthcare.
- Collaborate with your supervising physician on new and existing patients, addressing acute needs and medical complexities.
- Perform virtual assessments and provide medical guidance to the care team as needed.
Qualifications
- Current NP or PA license and certification.
- Graduate of an accredited program.
- Experience in Internal Medicine, Family Medicine, or Geriatrics.
- Excellent communication, active listening, and interpersonal skills.
- Strong analytical and critical thinking abilities.
- Compassionate, patient-focused approach to care.
- Comfortable with technology as communication is managed electronically with your support staff
