**Summary:**
Provides direct patient care to patients with wounds and ostomies using the
nursing process in accordance with applicable scope and standards of practice
and with the policies, values, and mission of the organization.
This position has availability for the fellowship program and a BSN is
require.
**Responsibilities:**
Evaluates learning needs of patient andor family and provides patientfamily
education appropriate to age, culture, condition, and
circumstances.Collaborates across disciplines to coordinate patient care,
including patient transfer, discharge, referral and spiritualpsychosocial
support needs.Effectively delegates patient care to ancillary personnel. May
precept student nurses and new hires.Plans, implements, and evaluates patient
care plans based on patient assessment to optimize outcomes and maximize
available resources. Monitors, records, and communicates patient condition as
appropriate.

Accredited Program: Nursing (Required)BLS American Heart E-Card (BLS AHA

Certification Grace (BLS GRACE) - Employee Grace Period for Essential

**Additional Job Information:**

One (1) year of nursing work experience required.Other Information: WOCN
