Responsible for the transition of care/discharge planning for both inpatients and our skilled rehab patients. Triages, coordinates and facilitates the clinical management for an assigned population. Develops, monitors and reinforces care standards and protocols designed to enhance quality and promote cost-effective utilization of healthcare resources. Manages the care of assigned patients from intake through discharge, and follow up. Coordinates the delivery of services to patients and families. Manages the financial resources of the patient by acting as the liaison to the payer source and assuring cost-effectiveness of the treatment provided by the team. Facilitates and coordinates team conference reporting and the discharge planning process. Serves as the patient and family advocate to coordinate and direct effective communication regarding the patient’s care. Performs post-discharge follow-up.
