Job Description
NurseCore is seeking a flexible and experienced
Registered Nurse to provide
intermittent home health visits for patients in the
post-acute stage who require ongoing skilled care at home, such as
wound care, catheter changes , or support with more complex needs including
ventilators, tracheostomies, or G-tubes . Approximately
5 to 10 visits per week , typically requiring
1 to 2 hours per day .
Visit Types & Pay Rates: - Start of Care (SOC) – $100. Initial admission assessment and care setup following hospital discharge.
- Resumption of Care (ROC) – $70. Re-starting services post-hospitalization.
- 60-Day Recertifications – $85 . Planned within a 5-day window.
- Wound Care Visits – $70. May take 2–3 hours depending on the complexity.
- Catheter Changes – $70
- Supervisory Visits (every 14 days) – $45 . Includes auditing, nurse evaluations, and assessments
Schedule & Hours: - Visits based on patient needs
- SOC and ROC visits are less predictable and often vary day-to-day
- Wound care and catheter visits can typically be coordinated directly with patients
- Most visits are after 7:00 AM, with some occurring after-hours or on-call
What you need: - Ventilator and tracheostomy certification is required for SOC, and ROC, and Supervisory visits
- G-tube experience is strongly preferred
- If not vent/trach certified, RN may still be considered for wound care and catheter visits only
- Prior case management experience is a plus
- Must be reliable, adaptable, and able to respond to evolving patient schedules
- Active RN license in Nevada
- Current hands-on CPR certification
- Negative TB results within the last 12 months
- Successful completion of background check
