RN - Maternal Health Case Manager

Responsibilities:

Provide case management and nursing care for clients during the prenatal and postpartum periods.
Conduct screenings for social determinants of health (SDOH), perinatal depression, and pregnancy/postpartum risk factors.
Develop individualized care plans and measurable goals with clients to monitor progress.
Educate clients on perinatal health standards and provide short-term interventions and emotional support.
Complete postpartum home visits within 30 days of delivery as needed.
Document all interactions and interventions in the electronic medical record (EMR) within one business day.
Attend required virtual training sessions, meetings, and workshops as directed by the supervisor.

Qualifications:

Proficiency in Microsoft Office and experience with EMR systems, preferably Epic.
